Models of Post-Treatment Care for Oncology Patients – a Literature Review

Managing the burden of cancer requires a holistic approach, which means integration at all levels of health care. The aim of the literature review is to identify the continuity of coordinated integrated holistic care for oncology patients after oncology treatment. A literature review was performed using the PubMed, MEDLINE, and WEB OF SCIENCE databases. The search was conducted using the following keywords: cancer care, rehabilitation, cancer survivors, follow-up, quality of life. The qualitative analysis included 18 scientific and peer-reviewed articles. The results identified four models of healthcare providers, which can be broadly classified into specialist oncologist-led care, care shared between a general practitioner and a specialist oncologist, general practitioner-led care, and nurse-led care. Care does not end at the end of treatment, but must extend to the long-term health needs of oncology patients. The integrated care model needs to be adapted to patient-level factors, local services, and healthcare policy to ensure optimal follow-up of oncology patients throughout the cancer care continuum.
